Inflammatory lung disorders (e.g. asthma, chronic obstructive pulmonary disease, interstitial lung diseases/sarcoidosis) affect a substantial part of the population causing considerable suffering and costs. The lung research unit focuses in various set-ups on unraveling mechanisms behind the initiation and progression of these conditions, some of us using sarcoidosis as disease model. A thorough identification of clinical subsets, studies of susceptible genes for disease prediction, longitudinal follow-up and unique access to the site of inflammation through bronchoscopic techniques enabled us to reach cutting edge knowledge on immunopathogenic mechanisms in sarcoidosis. New approaches to identify patients at specific risk have been developed, and thus we may now also by using novel treatment modalities avoid disease progression to disabling respiratory failure. The research is truly translational combining clinical investigations and findings with modern molecular biology techniques. Furthermore, the study design may enable discovery of disease causing agent(s) in sarcoidosis, and is a cornerstone in collaborative research with rheumatologists (RA, myositis) and neuroimmunologists (MS). The lung unit has managed to create possibly the largest healthy non-smoking and smoking control cohorts of the world of bronchoalveolar lavage samples providing valuable basic data.
